These problems often overlap but are not interchangeable. Treatment depends on whether the issue is skin, eyelid position, brow position, fat, hollowing, pigment, vessels or fluid.

Almost everyone arrives with the same complaint — looking tired — and that single appearance has at least eight different causes with eight different answers. This page exists to work out which one is yours before anything is treated.

Most people have two or three of these at once. That is the usual situation rather than the exception, and it is why a plan that addresses the dominant cause first often reduces how much else is needed.

The eyelid margin sits lower because of levator/aponeurotic, congenital, neurological or other causes. It requires a ptosis-specific assessment.

Brow descent

A low brow can push tissue downward and mimic eyelid heaviness. Brow lift is a separate surgical pathway.

Two checks you can do in a mirror at home narrow this considerably. Hold your brow gently in its natural resting position — if the heaviness noticeably worsens, brow descent is contributing. Relax your forehead completely, deliberately letting the brows fall; most people are surprised how much lower everything sits, and that relaxed position is the true starting point.

A warning worth reading before booking anything: if you are relying on unconscious brow lifting to see past a heavy lid, treating your forehead lines with botulinum toxin can make the heaviness noticeably worse. The lines are a symptom of the eyelid problem, not a separate issue, which is why an eyelid assessment should come first.

New sudden ptosis, double vision, unequal pupils or fluctuating eyelid weakness needs medical assessment before cosmetic treatment. A droop with a small pupil, a droop with a large pupil and double vision, and a droop that worsens through the day all point in different medical directions.

Fluid puffiness

Swelling that is worse on waking and settles through the day is fluid, not fat, and no procedure fixes it.

Skin laxity

Crepey lower-lid skin behaves differently from bulging fat and may need resurfacing rather than surgery.

Mid-cheek descent

Loss of cheek support deepens the transition and can make an eyebag look worse than it is.

A simple distinction that changes the plan: if the puffiness is worst when you wake and settles over a few hours, it is fluid — and surgery or filler will not help. If it is present all day and unchanged by sleep, it is more likely fat or hollowing, which are treatable.

Dark circles deserve their own separation because the word covers at least three different things. Brown discolouration is pigment. A bluish or purple tint is usually vessels showing through thin skin. A shadow that changes with lighting is structural — cast by a bulge above a hollow. Each has a different treatment, and treating pigment when the problem is shadow achieves nothing.

Skin quality around the eyes

Fine crepey skin, dynamic lines and pigment need different approaches. Skin boosters, fractional laser, RF, botulinum toxin and surgery should not be presented as interchangeable “eye rejuvenation” treatments.

The skin here is the thinnest on the body, which explains both why it shows change earliest and why it is the least forgiving area to treat aggressively. Energy settings, injection depth and product choice all need adjusting relative to what would be used elsewhere on the face.

One genuinely high-yield habit: sunscreen applied up to the orbital rim and sunglasses outdoors. Ultraviolet exposure drives much of the thinning, pigmentation and fine lining in this area, and in Singapore's year-round UV that protection does more over a decade than any single procedure.

Why diagnosis matters

Adding filler to a true eyebag can increase puffiness. Removing upper-eyelid skin does not repair true ptosis. Treating pigment will not correct a structural hollow. The same “tired-eye” appearance can therefore require completely different treatment.

Frequently asked questions

Why do I always look tired?

Because at least eight different things produce that appearance — heavy lids, low brow, ptosis, eyebags, hollowing, pigment, vessels and crepey skin. Identifying which applies to you is the whole point of assessment.

How do I tell if my puffiness is fat or fluid?

If it is worst on waking and settles over a few hours, it is fluid, and neither surgery nor filler will help. If it is unchanged by sleep, it is more likely fat or hollowing.

Are all dark circles the same?
